Appointment Questions

Do I Have To Go Into the Office for My Appointment or Can I Use Telehealth? 

In most cases, we are able to complete testing using Telehealth.

 

How Should I Prepare My Child for an Evaluation?

It is important to let your child know what to expect during the evaluation. They will be asked to answer a lot of different types of questions, depending on why they were referred and what is found during the evaluation.  In order to collect meaningful and reliable information from your child's appointment please do not attempt to "prep" for testing.  If your child is completing testing using telehealth, a quiet, distraction-free environment, is preferred.  Your child will also need a few sheets of paper and a pen.  A drink and a snack are also helpful to have on hand. Depending on your child's age and the purpose of the evaluation, your child may be asked to complete a questionnaire to gain a clearer picture of symptoms and strengths.  These questionnaires are emailed and can be completed after the evaluation.  It is a good idea to encourage your child to complete any questionnaires within a couple of days after the appointment.   

Do You Provide Therapy or Other Mental Health Services?

 

Dr. Duke's private practice is dedicated to neuropsychological evaluations.  At this time, she does not provide therapy at this office.  If you would like a referral for a psychologist or other mental health professional, let Dr. Duke know during your appointment. 

Who Should I Bring to My Appointment?

It is often very useful to gain multiple perspectives on your current level of functioning.  A spouse, parent, roommate, or close friend can be a useful person to bring to your appointment.  This person may be asked questions during the interview and/or may be asked to complete questionnaires. Typically this information is collected at the beginning of the appointment and takes around an hour.  

What Type of Conditions Do You Typically Diagnose?

 

A neuropsychological evaluation is normally recommended to determine if a person meets the criteria for a diagnosis. This clinic specializes in neurodevelopmental (ADHD, autism, learning disabilities, and developmental delay) and neurocognitive diagnoses (head injuries, dementia).  

How Long Will it Take?

Depending on why you are referred for an evaulation, testing will take between 3-8 hours. 

 

Is This Covered by Insurance?

Dr. Duke accepts most insurances including Medicaid, with the exception of Ohana. 

 

What Should I Bring?

 

  • Please bring your ID and Insurance Card.

  • Information from your referring provider, including imaging (MRI, CT scan). If you do not have them, please request a copy from your doctor and have them sent to my office. 

  • Individualized Education Plan's (IEP's) 

  • ​Water and a snack is recommended.
